This page is for families who have made a booking with Moor Time for the first time, and have been asked to complete a permissions form.
 
The form gives us essential information about whether we can use photographs of your young person, whether they require medication, if they need road safety support, who will be collecting them etc, so please complete it with care. 

It takes about 5 minutes to do. Once it's complete, we will print a copy for you to check and sign when you drop your young person off. If you would like us to post you a copy of your form before then, please let us know.
